Family Income Inquiry Form - Please complete today!
Please complete your Family Income Inquiry Form online today at myschoolapps.com. Our school is listed as 20K102. This form is used across the state to determine if your child qualifies for free or reduced price lunch. In the last several years, all NYC DOE schools have qualified for universal free lunch. The form is also used to determine Federal Title I funding. We did qualify for the Title I funding for the 2023/2024 school year. Completion of this form determines our funding for the 2024/2025 school year.
You only need to complete one form per family for all children in NYC Public Schools. We ask for 100% of our families to complete the form so that we have a chance to qualify again for next year. It is almost one million dollars in funding. Please include all financial information and add all family members living in the household. If you have a high income and are sure you would not qualify - please complete the form anyway - you have an option to check off that you do not qualify/will not be providing financial information. If you are receiving food stamps or state benefits you do not need to complete the form. If you have any questions on this form, please contact Ms. Sheri at melliotsheri@schools.nyc.gov. Thanks!
